Like this you can create different contact form for different purpose and they will be appended with numeric 2,3 and so on.
While making your required list for contact information, you can make some of them as optional and some of them as required.

There are many contact plugins and it’s hard to find the right one. I do my best to stay away from ones that use CAPTCHA, there are sometimes when it does not work.
I remember trying to leave a comment and it kept on saying that the code I was entering is incorrect. I eventually gave up.
What I would like to mention is the multiple forms, that’s a bonus if you need it for other details.

Nice Plugin but it does not provides more functionality than the Sites like JotForm and all which provide advance options. But Plugin is Nice.
